Weekly Practice Parent Sign Off Sheet

On lesson day, submit an accurate record of your student's practice throughout the week. Successful accomplishment of 3 or more days of practice per week (aside from lesson day) will be rewarded with 1 Practice Point. Every 10 Practice Points is awarded 1 Sparrow Coin.
